Fire report (in case of damage exceeding Rs. 2 crores);


Providing fire report based on the report of the investigation committee after making a written application with necessary documents by the affected person/institution;


Documents:

1. Application form (white paper);

2. Copy of GD;

3. List of damaged goods with value;

4. Still images of damaged goods;

5. paper cutting;

6. Original copy of invoice;
150/- and 1500/- in case of uninsured and insured institutions (15% VAT to be added in both cases) should be deposited through treasury challan to Bangladesh Bank/Sonali Bank in Code No.-1-7361-0000-2009; Notification of decision within 05 (five) working days of receipt of full investigation report;

(Note: Application will not be accepted if money is deposited through pay slip other than the pay slip downloaded from BRAC Bank online). The original copy of the downloaded pay slip should be attached with the application form. Institutional authorities will arrange transportation for training, consultation and survey;

6. Post Graduate Diploma in Fire Science and Technology

Bachelor's/Equivalent Degree in any discipline (but must have science background at higher secondary level)
